Primary Care – NHS Salaried Advanced Clinical Practitioner in Derbyshire
We are looking for an Advanced Clinical Practitioner to work in Derbyshire for on a permanent contract with either a full time or part time position considered.
The job will involve working alongside a multidisciplinary team, including GPs, Nurse Practitioners, Clinical Pharmacist and Nurses to provide care and treatment to patients with either long term conditions or undiagnosed undifferentiated diagnosis.
You will be supported by the Clinical Lead.
You will be involved in Practice meetings and have the ability to influence change and improve care for patients as well as the opportunity to lead on specific clinical based projects within the Practice team.
Post: Advanced Clinical Practitioner
Salary – £45,000 - £60,000
Start date: as soon as possible
Length of contract: to be confirmed
Required Qualifications:
- Masters or Equivalent
- Independent Prescriber
- NMC registration
Key duties and responsibilities
Clinical Practice
- Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
- Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
- Priorities health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Triage telephone call referrals from external clinicians, patients and others into a range of services appropriately as required utilising video consultation resources
