RGN/Intermediate Care Staff Nurse Band 5/6 in Warrington

GP World have a fantastic new opportunity for RGN/Intermediate Care Staff Nurse Band 5/6 in Warrington   

Hours:  This is negotiable

Start Date: - As soon as possible

End date – 6-month Contract

Rate: Negotiable  

Job Purpose:

  • The post holder undertakes responsibility for delegated delivery and evaluation of patient centred goals, including maintaining individual care plans to assist rehabilitation.

  • Ensuring a high standard of care with intermediate care bed-based facility.

  • Liaises with the Intermediate Care Sister / Charge Nurse to ensure information in relation to patient care/changes to care plan and the continuity of care.

  • Provides mentoring, teaching and assessing to support staff.

  • Liaises with all health care professionals and other agencies to ensure effective service delivery in the intermediate care facility.

 

Duties and Responsibilities

Clinical

  • Perform comprehensive assessment of patient nursing needs.

  • Assess, plan, implement and evaluate programmes of care according to changing health needs under the direction of the Intermediate Care Sister / Charge Nurse.

  • Maintain accurate records in accordance with NMC.

  • Attend a multi-disciplinary case conference as the named nurse if appropriate.

  • To communicate and collaborate with other members of the Integrated Intermediate Care Team, PHCT, voluntary and statutory organisations to support individual patient care.

  • To establish and maintain effective communication with patients, relatives and carers in complex and potentially challenging situations.

  • To competently perform clinical skills related to the post.

  • To be professionally responsible for delegated visits under the supervision of the Intermediate Care Sister / Charge Nurse

  • To work without direct supervision.

Managerial

  • To be responsible for work allocations of support staff.

  • To complete monthly supervisions for support staff.

  • To be responsible for organising and prioritising own caseload.

   

The post holder must:-

  • Take all reasonable care to minimise the risk to service users, colleagues and members of the public (including themselves), of healthcare-associated infections.

  • Co-operate with the Trust in ensuring that all statutory and mandatory requirements are complied with – in particular, compliance with the Health and Social Care Act 2008: Code of practice for the NHS on the prevention and control of healthcare associated infections and related guidance.

  • Be knowledgeable of and compliant with the Trust’s infection control policies and procedures, which include attendance at mandatory training.                                                                                                                                                                                                                                                         

 

Qualifications

Essential

  1. First Level Nurse

  2. Current Licence to Practice (or willingness to undertake return to practice

Desirable

  1. Diploma or Equivalent

Experience

Essential

  1. Post Registration experience

  2. Newly qualified Nurse with intermediate Care interest

Desirable

  1. Experience in intermediate care

Skills and Knowledge

Essential

  1. Range of extended clinical skills and ability to demonstrate knowledge

  2. Able to communicate unpleasant/sensitive information to patient’s relatives and carers

  3. Able to demonstrate organisational and managerial skills

  4. Able to develop effective interpersonal skills relationships across the health and social care setting

  5. Proven teaching and assessing skills verbal and written skills

  3. Venepuncture Course

  4. Wound Care Course
